The Clinical Coach
Differentiating Dorsal Impingement, TFCC Load Intolerance, and Extensor Tendinopathy in the Loaded Rack
T points to the back of his wrist first. Right in the center, over the dorsal capsule, just distal to the end of the radius. That is his main complaint. It lights up in the bottom of the front rack, when the bar sits on his shoulders and the wrist is jammed into full extension.
Load plus extension equals pain.
Then he adds the second piece. There is a different ache, this one on the pinky side of the wrist, that shows up during cleans and jerks when he has to turn the forearm, and during heavy carries. A deeper, more diffuse discomfort than the sharp dorsal catch. Continue reading …

Shiny Strategy Syndrome - Don’t fall for it.
When I completed my practice annual review in 2025, something was pretty obvious. The strategy I began 2024 was not the one I finished with. After going back through the months, it was pretty obvious that I abandoned that strategy 47 days in.
I couldn’t remember why. I dug deeper. I soon realized that I had fallen for the shinny object syndrome. In this case, the shiny strategy syndrome.
In February of 2024 I came across this growth strategy that seemed too good to be true. All of these Rehab Chiros and DPTs where used as examples. Some doubling their revenue in 15-30 days. It was a little too good to be true, but I was intrigued. I wasn’t doing terrible in business, but I had just started from scratch. I wanted to do things better than before.
45 days later, I was busier… but not in the way I wanted. I was doing more outreach to those in my niche. I e-mailed personal trainers, golf pros, athletic trainers, coaches, and more. I was doing more marketing than I had in the past. Did it work? Not quite. In those 45 days I had 8 new patients. When I compared it with the first 47 days of 2024. My new patients actually dropped (from 22).
My heart sank. WTF?
It took me a solid month to get out of that funk. If you are keeping track that’s now about 120 days. Almost three months wasted.
The next six months I took a different approach. Since I had already done all of that work, I decided I’d keep up with those who’d responded to my initial emails. This panned out to be an excellent move on my part. Took a few of them out to lunch, got to learn about their business, and since then we’ve referred a few clients back and forth.
Did my income double up in the next 6 months? Yes. But only because I began doing the right things, and did not jump into a different strategy just because it seemed better.
